What Heart Conditions Qualify for Disability Benefits?

1. Chronic Heart Failure (Listing 4.02)

Chronic heart failure qualifies when your systolic or diastolic dysfunction is documented and your symptoms persist despite treatment. The SSA looks for specific ejection fraction measurements combined with evidence of ongoing limitations in physical exertion, such as episodes of shortness of breath at minimal activity or fluid retention that doesn't resolve. Both systolic (reduced pumping function) and diastolic (stiffening of the heart wall) forms are evaluated.

2. Ischemic Heart Disease (Listing 4.04)

Ischemic heart disease, including coronary artery disease and stable angina, qualifies based on results from exercise tolerance testing or imaging that shows documented myocardial damage. The SSA specifically evaluates whether you experience chest pain at low levels of exertion, and what your VO2 max results show about your cardiovascular capacity. Results from cardiac catheterization and stress testing are central to this listing.

3. Recurrent Arrhythmias (Listing 4.05)

Arrhythmias that recur despite treatment and cause documented episodes of syncope (fainting), near-syncope, or significant cardiac symptoms can meet this listing. The SSA requires that the arrhythmia be confirmed by electrocardiogram or Holter monitor recordings. Beginning in October 2026, SSA will require confirmation by both electrocardiogram and Holter monitor recordings instead of one or the other. If your arrhythmia has caused you to lose consciousness or significantly limits your daily activities, this listing may apply.

4. Symptomatic Congenital Heart Disease (Listing 4.06)

Adults with congenital heart conditions that cause chronic cyanosis, recurrent arrhythmias, or exercise intolerance can qualify under this listing. The SSA evaluates oxygen saturation levels, documented complications, and the degree to which your symptoms limit physical activity. Beginning in October 2026, SSA will require evidence supported by hypoxemia rather than cyanosis.

5. Heart Transplant (Listing 4.09)

If you have received a heart transplant, you qualify automatically for disability benefits for at least 12 months following the transplant date. After that period, the SSA evaluates residual impairment from the transplant and any ongoing complications. This is one of the cleaner automatic qualifications in cardiovascular disability law.

6. Aneurysm of the Aorta or Major Branches (Listing 4.10)

Aortic aneurysms that are not amenable to surgery and cause documented limitations may qualify. The SSA evaluates the size of the aneurysm, surgical risk, and functional limitations.

7. Chronic Venous Insufficiency (Listing 4.11)

Chronic venous insufficiency of a lower extremity qualifies when it causes skin ulceration that hasn't healed after three months of treatment, or when it causes brawny edema (hardening of the skin tissue) with documented limitations in standing and walking. Medical documentation of venous imaging and wound care records is essential.

8. Peripheral Arterial Disease (Listing 4.12)

Peripheral arterial disease qualifies under this listing when imaging or Doppler studies show a specific reduction in ankle-brachial index (ABI) that correlates with significant functional limitation in walking.

How Hard Is It to Get Disability for Heart Disease?

Getting disability for heart disease is harder than most people expect at the start of the process, and easier than many assume after an initial denial. The SSA denies roughly 60 to 70 percent of initial applications across all conditions. Heart disease claims face a specific challenge: the SSA wants to see that your condition has been documented consistently over time, treated aggressively, and that you still have significant functional limitations despite that treatment.

Here's the thing: a denial on your first application is not the end of your case. It's the beginning of an appeals process where the odds shift substantially in your favor, especially at the ALJ hearing level. How to Apply for Heart Disease Disability Benefits in California

The application process has several steps, and where you start determines how long the road ahead will be. Most people file initially, get denied, and then figure out the appeals process. Filing correctly from the start, with the right evidence and the right framing of your limitations, gives your claim the best chance at each level.

1. Gather your medical records. Before filing, collect at least 12 months of cardiology treatment records, hospital discharge summaries, and any imaging or testing results. The SSA will request records, but having them organized from the start avoids delays.
2. File your initial application. You can apply online at SSA.gov, by phone at 1-800-772-1213, or in person at your local Social Security office. You'll complete a Disability Report (SSA-3368) and an Authorization to Disclose Information (SSA-827) for medical records.
3. California DDS review. Once SSA receives your application, it routes your file to the California Department of Social Services Disability Determination Services (DDS) for a medical review. A DDS examiner and medical consultant evaluate your records and make the initial determination. Processing times vary but typically run 3 to 6 months for an initial decision.
4. Consultative examination (if requested). If DDS finds your records insufficient, they may schedule a CE with an SSA-contracted physician. Attend this appointment. Failing to appear without good cause can result in a denial.
5. Receive your initial decision. Most initial applications are denied. If you're denied, you have 60 days from the date of the denial notice (plus 5 days for mailing) to file a request for reconsideration.

What to Do If Your Heart Disease Disability Claim Is Denied

A denial is not a decision. It's the start of an appeals process where the majority of ultimately successful disability cases are won. Heart disease claims that are denied at the initial level can be appealed through four stages, each with its own deadline and procedural requirements.

1. Reconsideration. Request within 60 days of your initial denial. A different DDS examiner reviews your case with any new evidence you submit. Approval rates at this level are significantly higher than at the initial or reconsideration levels, particularly for claimants with legal representation. In California, hearings for the greater Los Angeles area are handled by ODAR offices in Los Angeles, Pasadena, and Long Beach; Northern California claimants typically appear at Sacramento, Oakland, or San Jose offices.
3. Appeals Council. If the ALJ denies your claim, you can request review by the SSA's Appeals Council within 60 days. The Council may grant your case, deny review, or remand it back to an ALJ for a new hearing.
4. Federal District Court. If the Appeals Council denies review, you can file a civil action in the U.S. District Court for your district within 60 days. Federal court review evaluates whether the SSA's decision was supported by substantial evidence in the administrative record.

The hearing level is where most heart disease cases are decided. An ALJ can weigh your treating cardiologist's opinion against the SSA's consulting physicians, evaluate your credibility, and reach a different conclusion than the DDS examiner who never met you.
