Medicare is the federal health insurance program for Americans 65 and older, but workers who become disabled and qualify for SSDI also receive Medicare, regardless of age. The catch is you don't get it right away. There's a mandatory waiting period, and understanding how that timeline works is essential for planning your healthcare coverage after a disability approval.

When SSA approves your SSDI claim, your Medicare eligibility begins 24 months after your date of entitlement. Your date of entitlement is typically the month your SSDI payments begin, not the date SSA approves your application. Add the five-month waiting period SSA imposes before SSDI payments start, and the total time from your disability onset date to Medicare eligibility is often 29 months.

The 24-Month Waiting Period Explained

The 24-month clock starts on your SSDI entitlement date, the first month you're entitled to benefits after the five-month waiting period. SSA counts calendar months, not days, so a partial month at the start counts as a full month. If your entitlement date is July 15, SSA counts July as month one.

Here's how SSA calculates your Medicare start date:

1. Determine your established disability onset date (EOD)
2. Add five months; this is your SSDI entitlement date
3. Add 24 months to your entitlement date; this is when Medicare begins
4. Medicare coverage begins on the first day of the 25th month

SSA will notify you by mail when your Medicare eligibility approaches. You'll receive your Medicare card automatically; you don't need to apply separately for Part A. Part B requires a decision on your end, which we cover below.

Exceptions to the Waiting Period (ALS, ESRD)

Two conditions bypass the 24-month wait entirely. If you have Amyotrophic Lateral Sclerosis (ALS), Medicare begins the same month your SSDI payments start with no waiting period at all. If you have End-Stage Renal Disease (ESRD) requiring dialysis or a kidney transplant, Medicare eligibility begins three months after dialysis starts, or in some cases immediately.

These are the only statutory exceptions. Other severe conditions, including certain cancers on SSA's Compassionate Allowances list, receive expedited processing for SSDI approval, but the 24-month Medicare wait still applies after approval.

Medicare Part A vs. Part B for Disability Recipients

Part A covers hospital inpatient care, skilled nursing facility stays, hospice, and some home health services. If you've worked and paid FICA taxes for the required period, you get Part A at no premium cost. Most SSDI recipients qualify for premium-free Part A.

Part B covers outpatient care: doctor visits, diagnostic tests, preventive services, and durable medical equipment. Part B has a monthly premium. In 2026, the standard Part B premium is $202.90/month, though higher-income individuals pay more. You're automatically enrolled in Part B when your Medicare begins, but you have the right to opt out. How Medi-Cal (California Medicaid) Works with Disability Benefits

Medi-Cal is California's version of Medicaid, and for SSI recipients in California, enrollment is automatic. The state's "Medicare/Medi-Cal linkage" means that when SSA approves your SSI claim, the enrollment information goes directly to the California Department of Health Care Services (DHCS). You don't fill out a separate Medi-Cal application. Coverage typically begins the same month your SSI benefits start.

For SSDI recipients who don't receive SSI, Medi-Cal eligibility depends on income and assets. The interaction between SSDI income and Medi-Cal's limits is one of the most common sources of confusion for disability recipients in California, and one of the most consequential to get wrong.

Medi-Cal Eligibility for SSI Recipients

SSI recipients in California get Medi-Cal automatically. SSA sends enrollment data to DHCS, and Medi-Cal coverage begins with your first month of SSI eligibility. There's no gap, no application, and no fee. This is one of the most important protections California has built for its most vulnerable residents.

For individuals who don't receive SSI but have limited income, California has expanded Medi-Cal eligibility significantly under the ACA. Eligibility is based on Modified Adjusted Gross Income (MAGI). SSDI payments generally count as income for Medi-Cal purposes, which is why some SSDI recipients who receive modest benefits still qualify for Medi-Cal alongside Medicare.

Will My Income or Assets Cause Me to Lose Medi-Cal?

This depends on which Medi-Cal program covers you. For MAGI-based Medi-Cal, the primary test is income, not assets. California eliminated the asset test for most Medi-Cal programs in 2024, meaning savings accounts and property generally no longer disqualify you. However, the California Department of Health Care Services (DHCS) has announced reinstating the Medi-Cal Asset Limit to SSI federal resource levels effective January 1, 2027.

For SSI-linked Medi-Cal, your continued Medi-Cal eligibility generally depends on your continued SSI eligibility. If your income or living situation changes and affects your SSI, it can affect your Medi-Cal. If you receive SSDI income that pushes you above SSI limits, you may lose SSI, but Medi-Cal has its own eligibility rules and you might still qualify. What Does Dual Eligibility Cover?

When Medicare is your primary coverage and Medi-Cal is secondary, the two programs work together to reduce your out-of-pocket costs significantly. Here's what dual eligibility can provide:

• Medi-Cal pays Medicare Part A and Part B premiums (for "full dual" beneficiaries)
• Medi-Cal covers Medicare deductibles and co-payments
• Medi-Cal covers services Medicare doesn't, including long-term care, dental, vision, and hearing
• Prescription drug coverage through Medicare Part D with Medi-Cal covering most cost-sharing
• Access to California's integrated managed care plans for dual eligibles (Cal MediConnect in participating counties)

What Disabilities Qualify for Medicare Before Age 65?

Any medical condition that qualifies you for SSDI qualifies you for Medicare, there's no separate Medicare disability list. If SSA approves your SSDI claim, Medicare follows after the 24-month waiting period. The question is really: what does SSA consider disabling?

SSA evaluates disability through its Blue Book (the Listing of Impairments), which organizes qualifying conditions by body system. Conditions that meet a Listing are automatically considered disabling if the clinical criteria are satisfied. Conditions that don't meet a Listing can still qualify through an RFC (Residual Functional Capacity) assessment, SSA's evaluation of what you can still do physically and mentally despite your impairments.

SSA's Compassionate Allowances program fast-tracks certain severe conditions; currently, over 200 diagnoses including many rare cancers, ALS, and early-onset Alzheimer's. Fast-tracked approval still comes with the 24-month Medicare wait, except for ALS and ESRD.

The condition itself matters less than how it affects your ability to work. Turning 65 on SSDI raises a question almost every long-term disability recipient eventually asks: does everything change? The short answer is that your healthcare coverage stays intact, but your disability benefit is reclassified. Understanding the mechanics prevents surprises.

SSDI to Retirement Benefits Conversion

At full retirement age (FRA), currently 67 for people born in 1960 or later, SSA automatically converts your SSDI benefit to a retirement benefit. The amount stays the same. Nothing changes in your monthly deposit. The conversion is administrative and happens without any action on your part.

One thing to know: once your benefit converts to retirement, SSA no longer conducts Continuing Disability Reviews (CDRs) to verify your disability status. Your continuing disability review obligations end at that point, which many recipients find to be a significant relief after years of periodic reviews.

Does Medicare Change at Age 65?

Your Medicare coverage continues without interruption at age 65. There's no new application, no new waiting period, and no change in benefits. You're already in the Medicare system, and you stay there. If you had only Part A before age 65 because you declined Part B, you'll have a Special Enrollment Period at 65 to add Part B without a late enrollment penalty but talk to a benefits counselor before making that decision.

What does change at 65: Medicare becomes the standard program for everyone your age, which can affect how your supplemental coverage works. If you have a Medigap policy or a Medicare Advantage plan, review those at 65 to confirm they remain appropriate for your situation.

Medi-Cal Eligibility Changes at 65

If you're dual eligible before age 65, that status doesn't automatically end at 65, but the Medi-Cal program category you fall under may shift. California has an "Aged and Disabled" Medi-Cal program specifically for people 65 and older, or under 65 with a disability, who meet income and asset requirements.

SSI recipients should be aware that full retirement age doesn't end SSI, if your income and resources still qualify, SSI continues. But if SSDI conversion to retirement income affects your SSI calculation, report any changes to SSA promptly to avoid an overpayment situation.

There are options, and knowing them matters.

Do Disability Payments Count as Income for Covered California?

Yes. SSDI payments count as income for Covered California purposes. SSI payments do not count as income for Covered California eligibility calculations. This is a critical distinction that affects how you apply and what subsidies you're eligible for.

If you receive SSDI and your total household income falls between 138% and 600% of the Federal Poverty Level (FPL), you can purchase coverage through Covered California and receive premium tax credits to reduce your monthly cost. If your income is below 138% FPL, you'd qualify for Medi-Cal instead, which is typically better coverage at lower cost.

One nuance specific to California: during the Medicare waiting period, some SSDI recipients have income low enough to qualify for Medi-Cal outright. Your options during the waiting period depend on your prior coverage and household situation. Here are the main paths:

1. Medi-Cal: If your income qualifies, this is usually the best option. Free or very low-cost comprehensive coverage. Apply through BenefitsCal.com or your county DHCS office.
2. COBRA: If you had employer health coverage, COBRA lets you continue it for up to 18 months (or 29 months if you're disabled under SSA rules). Premiums are high, you pay the full cost plus an administrative fee, but it preserves continuity of care with existing providers.
3. Covered California: If SSDI income disqualifies you from Medi-Cal but you're below 600% FPL, premium tax credits make Covered California plans more affordable. Open enrollment runs November through January, with Special Enrollment Periods for life events including disability approval.
4. Spouse's employer plan: If your spouse or domestic partner has employer coverage, qualifying for disability is typically a qualifying life event that allows you to join their plan mid-year.
5. Medicare Savings Programs: If your income is low enough, you may qualify for a Medicare Savings Program (MSP) that helps cover Part B premiums once Medicare begins. This doesn't help during the wait, but it's worth applying as you approach your Medicare start date.
